MIDDLE SCHOOL MOVEMENT
Congratulations to our rising 6th graders! In preparation for transitioning to middle school, please fill out the Middle School Movement form below. If your child is going to a middle school outside of BCPS, you can use the form to indicate you will need a transfer pack. If your child is staying in BCPS, you will need to submit Residency Reverification documents:
- Lease, deed, signed settlement sheet, mortgage statement or real estate tax bill.
- Photo ID with matching address or work ID, school ID, shopper’s club ID, passport, etc.
- 3 pieces of mail with the lease or deed holder's name and address, date stamped with an issue date or statement period less than 60 days old. No due dates or dates from the future.

SHARED DOMICILE RENEWAL
Each year, our families enrolled under a Shared Domicile arrangement must submit a Shared Domicile Disclosure Renewal Form along with updated residency documents:
- Photo ID with matching address or work ID, school ID, shopper’s club ID, passport, etc.
- 3 pieces of mail with the lease or deed holder's name and address, date stamped with an issue date or statement period less than 60 days old. No due dates or dates from the future.
